Self Directed Support Assessment

We recently asked for ideas about what to call the form that services users, along with a member of Nottinghamshire County Council staff, complete as part of getting a Personal Budget (see the post from April 24th, 2009).

The feedback we got gave us a clear winner, and the form will be called a “Self Directed Support Assessment”.

Self Directed Support Assessment Questionnaire

The first stage in getting a Personal Budget is an assessment. During the consultation and trial period we have called this a Self Assessment Questionnaire or “SAQ”.

In Nottinghamshire the questionnaire is filled in by the service user and a member of staff from Nottinghamshire County Council. So, we need a new title for the form.

Below is a list of the suggested titles - it would be helpful if you could comment on these and let us know what your top 3 choices would be. If you have any alternative suggestions, then please let us know.

We will need responses by 15th May 2009.

Vision Statement

The vision statement for Self Directed Support is:

We will place citizens of Nottinghamshire at the heart of a reformed social care system which will enable them to have greater choice and control over their lives.

We believe that every individual should be able to:

  • Live as independently as possible
  • Make their own choices to achieve their personal goals and aspirations
  • Take appropriate risks
  • Live their lives free from abuse and neglect
  • Maximise their health and well-being.

We will work in partnership with service users, carers and other involved agencies to ensure that high quality and personally tailored services are delivered by putting the individual at the heart of their assessment experience and the introduction of Personal Budgets.

We believe that this will offer people more choice, flexibility and control in achieving their desired outcomes.
