Self Directed Support Assessment Questionnaire
by Rebecca - April 24, 2009 – 1:17 pmThe first stage in getting a Personal Budget is an assessment. During the consultation and trial period we have called this a Self Assessment Questionnaire or “SAQ”.
In Nottinghamshire the questionnaire is filled in by the service user and a member of staff from Nottinghamshire County Council. So, we need a new title for the form.
Below is a list of the suggested titles - it would be helpful if you could comment on these and let us know what your top 3 choices would be. If you have any alternative suggestions, then please let us know.
We will need responses by 15th May 2009.
